Be Prepared and Make a List of Your Concerns-Before going to the provider make a list of what you want to discuss. Bring all items that may be needed, such as insurance cards, names of other providers, your medical records if new to the clinic, and list of medicines you are currently taking. Also bring co-payments for office call fees.

Ask Questions-Asking questions is the key to getting what you want from the visit. If you don't ask questions, your provider may think you understand why he or she is sending you for a test or that you don't want information. Ask questions when you do not know the meaning of a word, or when instructions are not clear. It may help to repeat back in your words what you think the provider is trying to explain.
Share Your Point of View-Your provider needs to know what's working and what is not. Your provider can not read your mind, so it's important that you share your point of view. Tell the provider if you feel rushed, worried, or uncomfortable. Try to voice your feelings.
Share Your Medical History-Tell your provider about your illnesses, operations, or medical conditions that run in the family. Tell your provider what other providers you see, what medications you're taking, including over-the-counter and herbal substances.
Occasionally your provider may need to deal with an emergency room visit. We will do our best to keep you informed as the situation warrants, and allow you to wait or choose to be rescheduled.
